Individualised In Home Supports

Our friendly support workers can provide individualised supports within your own home, assisting you with personal-care, meal preparations, activities of daily living, domestic assistance, night-time supports, and more.

Our friendly support workers provide individualised supports within the community, assisting you with social outings, transport, shopping, attending appointments, and more.

Life Skill Development

Our team will work with you and your family or carer to understand your goals first and foremost to develop a tailored support program to help develop everyday skills you require through activities to help increase your ability to live as autonomously as possible, with a focus on building your confidence and independence.

Support Coordination

Our friendly and experienced support coordinators are focused on working with you to optimise your plan so that you get the most out of your funded supports. We can guide you through the NDIS process from start to end.

Psychosocial Recovery Coaching

Our recovery coach can support you with your recovery. Recovery means being able to live a purposeful and meaningful life.

Psychosocial recovery coaches support individuals with psychosocial disability to recognise, reflect and take responsibility of their own recovery.

They work collaboratively with individuals, their families, carers and other services to design and implement a recovery plan and help with the coordination of other NDIS supports.

Individuals can choose to work with a recovery coach who has learned or lived experience of psychosocial disability.

Individualised In Home Supports

MacVincent Support Services is all about supporting participants to establish, work towards, and achieve their goals. Supports are provided with a person-centred approach, allowing flexibility and choice of individual supports received.

The focus of Individualised In-Home Supports is to support with activities within the home, such as:

  • Support with meal preparation
  • Basic domestic task support
  • Personal care needs
  • Supporting participants whilst their carer is otherwise unavailable
  • Providing participants with overnight support
  • Active awake night support
  • Supporting with recreational leisure activities of choice

Community Participation

MacVincent Support Services is all about supporting participants to establish, work towards, and achieve their goals. Supports are provided with a person-centred approach, allowing flexibility and choice of individual supports received.

  • Transport and Travel – Planning your journey, accessing public transport to help you get to where you need.
  • Shopping – Shopping is a great way to interact in the community, improve communication and social skills, as well as learning important life skills such as money handling.
  • Attending Appointments – We can provide you with transport assistance to GPs, dieticians, physiotherapists, psychologist and any additional support services you may require.
  • Attending Social Activities – Engage in community, social and/or recreational activities such as joining a social group, visit your local library, attending community events or joining a sporting club or even taking a Holiday.

Life Skill Development

  • Travel and Transport – Planning your journey, accessing public transport and travel training.
  • Budgeting – Develop a basic understanding of money handling skills through real life application and General money management skills including creating a budget and managing a bank account.
  • Employment – Support with Preparing a resume, Attending interviews, Workplace support, Health and Safety practices

Support Coordination

Support Coordination is NDIS funded support designed to help you to make the most of your NDIS funds. Once your plan has been approved, you can work with a support coordinator to help you understand and implement your plan. Our focus is to work with you in optimising your plan so that you get the most out of your funded supports. A support coordinator can guide you through the NDIS process from start to end including:

  • Working together to identify goals
  • Connecting you with community, mainstream and other government services
  • Selecting suitable support service providers based on your needs
  • Budgeting for each type of support required and negotiations with service providers
  • Ensuring service agreements and service bookings are completed
  • Assisting you with preparing for your NDIS plan review

Psychosocial Recovery Coaching

A recovery coach is an NDIS funded worker that has mental health knowledge. A recovery coach will:

  • Spend time with you, and people important to you, to get to know you and understand your needs
  • Help you to find out about different services and supports, and how these can help you
  • Help you get support from mental health services
  • Help you better understand the NDIS and support you with the NDIS
  • Develop a recovery Orientated relationship
  • Increase awareness of choice and control
  • Identify individual goals and develop strategies to achieve them
  • Provide ongoing support and engagement (even in times of increased needs due to variation in individual mental illness)
  • Build individual capacity, increase recovery skills, resilience and decision making.
  • Alcohol and Other Drugs Counselling, referral and support.

 

You can choose a recovery coach with lived experience. A recovery coach with lived experience has their own lived experience of mental ill health and recovery and are able to use this experience to inform their work.
